OHIP error code TM6
Registration not in effect on Service Date
What TM6 means
The Ministry of Health’s official wording for TM6 is: “Registration not in effect on Service Date.”
This code appears on your Claims Error Report (downloaded through MCEDT). It means the claim was rejected at submission and was never adjudicated for payment — fix the issue below and resubmit the claim.
How to fix TM6
Your telemedicine/virtual care registration wasn't in effect on the service date - confirm your registration status with the Ministry and ask whether a retroactive effective date is possible; resubmit only if your registration covers the service date.
Program-specific codes come from claims routed through special programs — Reciprocal Medical Billing for out-of-province patients, WSIB, telemedicine/virtual care, and Integrated Community Health Services Centres. The fix depends on that program's own submission rules.
