The Ministry of Health’s official wording for V65 is: “Missing master number - SLI code HDS, HED, HIP, HOP or HRP is included with a diagnostic service billing but a master number was not included.”
This code appears on your Claims Error Report (downloaded through MCEDT). It means the claim was rejected at submission and was never adjudicated for payment — fix the issue below and resubmit the claim.
How to fix V65
Add the master number (identifying the facility) to the SLI diagnostic-service claim and resubmit.
