How to fix HB
OHIP shows this was already paid - confirm it is not a true duplicate; file a Remittance Advice Inquiry with detail if the service was genuinely separate.

Where you disagree with the outcome, the usual route is a Remittance Advice Inquiry or written inquiry — not resubmitting the same claim, which can create a duplicate.
